Our Services

We offer the following services to our clients:

  4. Plan of Care Supports & Services (PCSS)
  5. Consolidated Supports & Services Brokerage (CSS)

At CLAS, we believe that our clients should choose which services, agencies and activities they would like to be involved with. CLAS, Inc works creatively with each person to discover the resources available to best meet individual needs.

 INDEPENDENCE 

CLAS is one of the few agencies in the Capital District that provides independent, autonomous case management because we do not provide other habilitation services. We can truly represent our clients without being beholden to any other agency or service provider.  

Because of our independence and autonomy from other service providers, we have the ability to truly act on behalf of our clients's best interests. We at CLAS, Inc. can act as an advocate for the people we serve.  

1. Intake & Referral:

Contact Nora Osuchowski at 598-9222 to discuss how to access the services offered by CLAS, Inc. Payment for services is arranged through Medicaid and private pay.

2. Medicaid Service Coordination (MSC)/Case Management:

We listen to your dreams and we help you develop an Individualized Service Plan (ISP) to make those dreams come true. We help to connect you to a variety of services and supports with in the community:

  • Community Outreach and Advocacy
  • Employment and Training opportunities
  • Community Habilitation
  • Day Habilitation Services
  • Residential opportunities
  • Recreation

CLAS, Inc works creatively with each person to discover the resources available to meet individual needs. 

3. Family Education & Training (FET):

 

We can provide information to families to increase knowledge in a wide array of topic areas to assist families in understanding and coping with developmental disability and its challenges.  See our calendar of events for more information.  

4. Plan of Care Supports & Services:

PCSS providers (a) assist consumers to review and update the ISP in accordance with applicable New York State regulations, federal guidelines and OMRDD policy, and (b) ensure the annually required level of care (LCED) determination is completed within one year of the previous determination. PCSS is delivered by a qualified Medicaid Service Coordinator.

 5. Consolidated Supports & Services (CSS): 

CSS is a completely person centered program where individuals work with their team (or 'circle of support') to develop an individualized and all encompassing program of supports. Each participant's program is unique to the individual for which it is created. CLAS provides brokerage services to assist with the development and maintenance of these plans.
